26-Hour Appointment Reschedule or Cancellation

It is important to schedule your next appointment during a date and time that you are able to keep. This will help you to stay on track with our therapeutic work and prescription refill schedule. Appointment rescheduling or cancellation that does not occur on the portal within 26-hours of your scheduled appointment time will result in the collection of the full fee for your appointment. 


Payment

I accept major credit cards. I do NOT participate in any insurance programs and you will be responsible for the full cost of all services rendered at the time of each service. A current credit card is required to reserve an appointment, but it will not be charged until the time of service or should an appointment be canceled with less than 26-hours notice.

Prescription Refills

You will always receive enough medication or refills to last until your next recommended follow-up. Should you run low or run-out, and not see availability for your appointment type on my online schedule, please schedule an Urgent / After Hours Appointment. Your well-being is important. Please schedule your appointments well in advance within the recommended date range to avoid running out of medication and the need for an urgent appointment.  

Good Faith Estimate, SERVICE PRICING, and office PolicY DOCUMENT

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. You may review the full office polices, including all service pricing, by clicking here.

A “Good Faith Estimate” will be provided in advance of scheduling if requested or it will be provided in the patient portal within 2 business days of scheduling.
